The issue in this grievance is whether or not the language contained in Part 513.364 of the ELM means the document itself must be generated by the doctor. During our discussion, we mutually agreed that the following constitutes full settlement of this grievance: The Employee and Labor Relations Manual contains no prohibition against the submission of a pre-printed form; however, it is understood that any medical documentation or other acceptable evidence submitted must meet the requirements set forth in Part 513.364 of the ELM.
