Return to Contract DatabaseThe question in this grievance is whether management may properly require an employee, who is involved in an accident, to complete the locally devised Accident Prevention Inquiry Form. During our discussion, it was mutually agreed that the following would represent a full settlement of this case:
The completion of the local form by an employee shall be voluntary. However, an employee may be required to answer the questions verbally. Such information can then be documented by the manager on PS Form 1769.
